AAIS currently offers form PU 0030. This Declarations page, when completed, provides the following account information:
· The name and address of the insurer (usually with a company logo)
· Policy number
· Producer's name and mailing address
· Name and mailing address of the insured
· Policy period including effective and expiration date with month, day, and year, as well as time of day
· Limit of insurance (for liability coverage and a retention)
· Premiums
· Schedule of primary (underlying) coverages and limits

· List of the forms and endorsements that apply to the umbrella
In addition to the PU 0030, AAIS makes form PU 0031, Personal Umbrella Liability Coverage Schedule of Underlying Insurance available. It contains areas for entering information regarding:
· Underlying coverages for personal liability (including personal injury), personal auto, commercial auto, employers liability, recreational vehicle, and watercraft liability
· Entering applicable limit options that exist in the underlying coverages – the options may be in the form of combined single limits or (according to coverage), separate per person or per incident limits for BI and PD
